Provider Resource
Welcome! If this is your first time accessing your patient’s health claim information or verifying eligibility, you will need a username and password to log in.
How to check your claim status
For your first login, please enter the following:
- Username: Your Provider Federal Tax ID (TIN)
- Temporary Password: Your Provider Federal Tax ID (TIN)
After signing in, you will be taken to the Health Claims Information screen where you can:
- Create a new password
- Update your login information
- Add or update your email address
Important: Be sure to provide a current email address. This will allow us to send password reset instructions if you forget your password in the future.
If your information is not found in our provider records, please contact LewerMark Student Insurance and submit a W-9 form for verification.
